Work location:Texas Health Denton 3000 N I-35 Denton TX 76201 (Onsite)
Work hours: PRN (As Needed) schedule can vary depending on the unit; Monday-Friday 8:30am-5:00pm or 10:00am-10:30pm and Saturday & Sunday 8:30am-5:00pm. This position requires 2 weekend days/month minimum and 1 critical holiday staffing.

Heres What You Need
Masters Degree Social Work is required upon hire. (Individuals hired as a CTSW prior to May 11 2017 will be grandfathered to the CTSW position with BSW at the entity they were employed at on May 11 2017)
Three (3) years of hospital/medical social work experience is preferred.
One (1) year of discharge planning/care management experience is preferred.
Licenses and Certifications
LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker is required upon hire.
CPR - Cardiopulmonary Resuscitation is required upon hire.
ACM - Accredited Case Manager is preferred upon hire.
CCM - Certified Case Manager is preferred upon hire.
Other ANCC is preferred.
In addition to the qualifications a successful Care Transition Manager - Social Worker - will model the factors listed below:
Working knowledge of medical necessity criteria preferred
Knowledge of Microsoft Outlook and Office (Word Excel)

Ability to engage in complex clinical decision-making
Strong oral and written communication skills
Strong commitment to interdisciplinary collaboration
Critical thinking analysis and conflict resolution skills
Psychosocial and crisis intervention skills
Ability to prioritize and meet deadlines
