Position Summary:

Position Accountabilities and Performance Criteria:
Ensures the welfare safetyhealthand rights of the individuals we serve.
Participates in the ISP process for the individuals we serveas requested.
Ability to follow a prepared schedule and routine foreach individualwe serve.
Ability to work flexible hours to accommodate staffing needs via On-Shift. Abilityto deploytoother departmentsto fill staff vacancies as directed by supervisor.
Operatesandmaintainsproperty and equipment safely and in good repair.
Superviseassistsand teaches individual personal care skills as asrequired.
Assistsindividuals to prepare and serve threenutritious meals theprepared menus whererequired.
Engagesthe individualswith daily chores activities and programs at all times.
Observesindividuals activities documents suchandreportsunusual behavior and/or occurrences to individuals work through conflict with others living in the Adult Interim Care Home.
Models age-appropriate andsocially according to the activities of the day or asdeemedappropriate bytheManager.
Displays an investment in the concept of normalization and an understanding of the agencys mission.
Transports the individuals we serve to and from community activities for the purpose of recreation leisure and medical appointments as well as keeping individuals informed of activities of interest.
Maintainsand dispenses required medications and treatments internal and external with utmost care and accuracy as directed by physicians orders andin accordance withMedicaid andWaiverregulations.
Attends on-going scheduled training sessions in-service programs and designated staff meetings andISPmeetings if requested by members of the team (CPIPositive and Person-Centered Culture& Delegated NursingCat I II and IIIcompleted within the first six months).Will completeDoDDonline training by the time they reach the equivalent of 2 yearsfull timeexperience.
Ensures confidentiality of written and verbal informationpertaining tothe individual and his/her family.
Maintainsaccurateand complete records of individuals personal possessions.
Assistsindividuals as needed with financial matters;ensurereceiptsare collectedandsubmittedtimely.
Documents all servicesprovided to the individuals we servetimelyand observations and interactions with individuals to Adult Interim Care Home management.
Is accountable for reportinganddocumenting MUIs andUIsina timelymanner.
Ensureshomeis clean and orderly with regard to premises equipment and furnishings. Tasks include but are not limited to windows floors toilets and walls.
Maintainsinterior and exterior of agency vehicles in a clean and orderly vehicles in a safe manner.
Notifies Managementtimelyforemergenciesequipmenthomeand major vehicle to handle communications withother providers lawenforcementand Montgomery County Board of Developmental Disability.
Adhere to all policies and procedures of the organization and all federal and state lawsregardingthe privacy of protected health information.
Adhere to all policies and procedures of the organizationregardingthe use of technology and technology-based information.
Assists Home Manager inpurchasingsupplies for the house and the individuals includinggroceriesetc.
Monitorsquantity of medications available forneedsof the individuals weserveand orders refills as needed.
Maintains vehicle upkeep (6 monthtune-up monthly oil checks & tire rotations etc.).Ensures regularly scheduled preventative maintenance tasks are complete. Operates vehicles in a safe manner.
On scheduled daysassistsHome Manager by being on call for scheduled length of time.
Oversee intakes as scheduled- this includes assisting individuals and families with intake packets leases completing inventories and going through medications.
Schedule and transport residents to medical appointments as directed ensuring all documentationis providedfrom each medical appointment.
Train new employees as directed. Mentor Care Specialists in the home as needed and as solves issues and resolves conflicts not requiring immediate management concerns to the HomeManager timely.
Dispose ofnon controlledextra medications kept in the managers office monthly. Both Team Leads will work together on this task.
Assistwith completion of weekly and monthly paperwork as directed.
Performs other duties as directed.
Position Qualifications:
Minimum Qualifications: Must be 18 years of ageor olderand haveHighSchool diploma be able to read and write in the English language to complete their job duties complete First Aid and CPR traininguponhireand prior to entering ability to relate to individuals in a meaningful and empathetic be willingableto work with persons whomaydisplay severe destructive behavior such as self-injurious behavior (SIB) aggression property destruction noncompliance tantrums elopementand other challenging behaviors andbe able to implement behavioral be willing to work as part of the Interdisciplinary have a valid drivers license anda good drivingrecord.
Minimum Experience:Twoyearsof developmental disabilityor mental healthexperience in residentialsetting.

TYPE A PHYSICAL REQUIREMENTS
ENVIRONMENTAL CONDITIONS:
Temperature ranges from 65-75degrees and may fluctuate from room to is variable ranging from bright to near darkness depending on patient/resident level varies from quiet to loud depending on area equipment and patient/resident needs.
PHYSICAL REQUIREMENTS:
Frequent bending and kneelingisrequiredwalking sitting and standing is involved 33% or more of the to kneel on the floor and effectively administerCardio PulmonaryResuscitation (CPR) in a manner safe to self and clients and todemonstratethis ability during CPR lifting isrequiredup to 50 lbs. of force occasionally and/or 20 and/or up to 10 lbs. constantly to move for exposure to heavy weight bearing > 150 lbs. alone in suddenemergency situations.
VISUAL REQUIREMENTS:

HEARING REQUIREMENTS:
Must have no hearing deficit related to use ofphone orperson to personcommunication.
