Employment Status: Full-Time 35 Hours per Week (Hybrid)
Reports To: Program Director and/or Assistant Director
Funding Source: NYC Aging
Salary Range: BSW/BA/BS Degree ($61k-66k)
Neighborhood Self Help by Older Persons Project (SHOPP) is a community-based nonprofit organization whose mission is to enhance the strength and resilience of individuals regardless of age or abilitythrough innovative programs and supportive services.
Program Description:
The SHOPP Caregiver Support Program (CSP) provides services to:
Adults 18 and over providing unpaid care to a family member or friend aged 60 and over.
Adults 18 and over providing care to anyone of an age with Alzheimers disease dementia or
related disorders.
Older adults 55 and over caring for:
An adult (18 59) with a disability.
Children or grandchildren (under 18) in kinship caregiving arrangements.
Position Summary
Under the supervision of the Program Director and/or Assistant Director the Care Specialist isresponsible for delivering caregiver support services in compliance with NYC Aging standards.
Responsibilities include client intake and assessment care planning case management caregivercounseling education respite coordination outreach documentation and reporting.
Duties and Responsibilities
*Maintain a caseload of ongoing and short-term caregiver clients in accordance with program guidelines.
*Conduct client intakes assessments reassessments and event-based screenings.
*Develop implement and monitor individualized care plans.
*Conducting ongoing service need assessments and ensuring timely follow-up.
*Provide supportive counseling and caregiver stress-reduction interventions.
*Conduct home visits monitoring calls and fieldwork as required.
*Screen for and coordinate in-home and out-of-home respite services.
*Provide caregivers with information and assistance regarding: Medicaid Long-Term Care Benefits and Entitlements Long-term care options (homecare adult day services nursing facilities etc.)
*Educate caregivers and care receivers on advanced directives including health care proxy power of attorney and guardianship.
*Provide advocacy referrals and follow-up assistance.
*Facilitate caregiver support groups and assist with educational workshops.
*Contribute to public education and caregiver training activities.
*Available for late office hours one evening per week to accommodate working caregivers.
*Participate in outreach activities community events and presentations including those outside normal business hours.
*Develop and maintain partnerships with community-based organizations and service providers.
*Assist with establishing and maintaining community-based service hubs throughout the Bronx.
*Maintain accurate and timely client records service documentation and data entry in NYC Aging designated databases.
*Prepare and submit required reports in accordance with NYC Aging timelines and requirements.
*Collaborating with program development implementation and special events.
*Provide support to interns and volunteers.
*Maintain current caregiver resource and referral information.
*Communicate effectively with supervisors and staff and meet all required deadlines.
*Participate in required meetings training and professional development activities.
*Foster a cooperative and supportive team environment.
*Adhere to all SHOPP policies NYC Aging program standards and contractual requirements.
*Perform additional duties as assigned by the Program Director or Assistant Director.
Minimum Qualifications
Bachelors degree in Social Work or a related field is required along with a minimum of two (2) years of relevant experience.
Work Schedule and Conditions
*Hybrid work schedule; staff must report to the office a minimum of 2 3 days per week or as
assigned.
*Ability to conduct home visits including walking up and down stairs.
*Ability to perform fieldwork in varying weather conditions.
*Required attendance at in-person meetings trainings and program events.
