Job Summary:

We are seeking a highly motivated and organized PAS Care Manager to join our this role you will play a crucial role in coordinating patient care collaborating with healthcare professionals and ensuring optimal patient outcomes. This position requires strong communication organizational and problem-solving skills as well as a passion for patient-centered care.

Responsibilities:

Patient Care Coordination:
Collaborate with physicians nurses and other healthcare professionals to develop and implement comprehensive patient care plans.
Monitor patient progress identify potential problems and adjust care plans as needed.
Provide education and support to patients and their families regarding their health conditions and care plans.

Communication and Collaboration:
Maintain clear and effective communication with patients families and healthcare team members.
Facilitate communication and collaboration among different departments and disciplines.
Participate in team meetings and rounds to ensure coordinated care

Documentation and Record Keeping:
Maintain accurate and up-to-date patient records and documentation.
Ensure compliance with relevant regulations and standards.

Resource Management:
Identify and access appropriate resources for patients and their families.
Advocate for patient needs and ensure access to necessary services.

Other Duties as Assigned:
Perform other duties as assigned by the supervisor.

Qualifications:

At least two years of experience in care management healthcare or home care settings is preferred.
Licensed Vocational Nurse
Social Worker.

Skills:

Strong communication interpersonal and organizational skills.
Ability to work independently and as part of a team.
Proficiency in electronic health record (EHR) systems.
Knowledge of relevant healthcare regulations and standards.
Empathy compassion and a patient-centered approach.

Compensation: $20.00 - $25.00 per hour
