Job Summary

Job Title: Care Manager OPWDD Services
Location: NY (Hybrid/Community-Based)
Job Type: Full-Time

We are seeking a compassionate and organized Care Manager to support individuals through OPWDD services in a person-centered care environment. This role focuses on care coordination advocacy community support and helping individuals access the services and resources they need.

Responsibilities:

Coordinate individualized care plans and member services
Conduct assessments and required follow-up visits
Connect individuals with medical behavioral health and community resources
Support transitions of care after hospitalization or rehabilitation
Collaborate with providers interdisciplinary teams and families
Maintain accurate documentation and HIPAA compliance
Assist individuals with benefits and community support services

Requirements:

Bachelors degree with 2 years of relevant experience OR
Active RN license with 2 years of relevant experience OR
Masters degree with 1 year of relevant experience
Strong communication organizational and computer skills
Experience in care management OPWDD behavioral health or community health preferred
