Care Manager ED (000087)

Skills:

• Five (5) years minimum of clinical experience as a Registered Nurse.
• Experience with Care Coordination Utilization review and discharge planning Computer literacy.
• Leadership experience. - preferred

Education:

• Associates Degree (ASN ADN) from an accreditated Nursing Program.
• Bachelors or Masters in Nursing. - preferred

Required Certifications & Licensure:

• RN Licensed as a Registered Nurse in the State of New York.
• PRI and Screen certification within three months of employment.
• Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
• BLS.
• Case Management Certification. - preferred
