Connected Crew Health LLC is a behavioral health and community-based services organization serving Charlotte and surrounding North Carolina communities. Our work focuses on providing comprehensive support including behavioral health services therapeutic youth programming peer support case management workforce development and more. As a Care Manager / Case Manager you will play a pivotal role in supporting individuals and families by connecting them with the resources and services they need. Youll work closely with youth adults and families facing behavioral health challenges housing instability justice involvement and other barriers to independence. Your work will help empower them to navigate lifes challenges and achieve their personal goals.

In this role you will:
- Provide case management services tailored to the unique needs of individuals and families.
- Collaborate with clients to develop personalized care plans that promote stability and growth.
- Connect clients with clinical educational housing workforce and community resources.
- Coordinate transportation crisis-related support and other essential services.
- Serve as an advocate for clients ensuring they have access to the support they need.
- Maintain accurate and confidential client records.
- Work collaboratively with a team of professionals to deliver high-quality services.

Were looking for someone who has:
- At least 3 years of experience in case management social work or a related field.
- A strong understanding of behavioral health and community-based services.
- Excellent communication and interpersonal skills.
- The ability to work effectively with diverse populations including youth adults and families.
- A passion for helping others and a commitment to our mission of promoting stability and independence.
