Care Coordinator (LPN) - Always Best Care
Are you a compassionate LPN who loves working with seniors and building meaningful relationships Always Best Care is seeking a dedicated Care Coordinator to join our growing team serving Boulder County Larimer County Weld County and Adams County.
In this role youll be responsible for conducting client assessments supervisory visits care plan development caregiver support and care coordination. Youll work closely with clients families caregivers and health care professionals to ensure every client receives exceptional service and support.
What Youll Do
- Conduct client assessments and supervisory visits
- Developand update individualized care plans
- support and coach caregivers in the field
- Investigate and document incidents
- Coordinate care among families and healthcare providers
- Ensure compliance with state regulations and company policies
- Build strong relationships with clients and their families
What Were Looking For
- Active Colorado LPN License
- Minimum 1 year of senior care experience
- Strong communication and organizational skills
- Experience working independently in the field
- Passion for helping seniors remain safe and independent
