HomeWell Care Services is looking for a compassionate organized and dependable Care Coordinator to join our growing team serving individuals and families across Duval Nassau and Clay Counties.
This is a part-time position with real full-time potential as we grow. You will work directly with our Agency Directors in a supportive fast-moving environment where your work genuinely makes a difference every single day.
What You Will Do
Be the go-to person for caregivers and families keeping communication clear and care consistent
Manage scheduling and step in to cover shifts in the field when needed
Keep client records and care plans current and accurate in WellSky
Support onboarding compliance tracking and day-to-day operations
Flag concerns to our supervising RN and Agency Directors and help find solutions fast
What We Are Looking For
Experience in home care or home health whether as a caregiver scheduler or coordinator
Active HHA or CNA certification
Comfortable stepping into the field when the team needs you
Familiarity with WellSky is a plus but not required
Proficient in Microsoft 365 including Word Outlook and OneDrive
A warm professional presence with strong people skills and genuine compassion
Organized reliable and able to juggle multiple priorities without losing focus
Valid Florida drivers license and reliable transportation
Must pass a Level 2 background screening

This is not a dead-end position. As you grow with us the path leads directly into a full Care Manager role with greater responsibility and compensation to match. We are building something and we want the right people building it with us.
