Sentara Health is hiring a Full Time Care Coordinator for the Roanoke VA area!
Hours: Full Time (40 hrs/wk)
Shift: Day (8am-5pm)
This position requires travel for in person face-to-face assessments in RoanokeVA . Days without assessments scheduled employee can work remotely.

Must possess a Bachelors degree in Health & Human Services or one of the following related Fields:

-Behavioral Sciences

-Cognitive Sciences
-Community Mental Health Counseling (MH Vocational Pastoral etc.)

-Early Childhood Development
-Educational Psychology
-Gerontology
-Healthcare Administration

-Nursing
-Pharmacy
-Psychiatric Rehabilitation
-Psychology

No specific certification or licensure requirements
1 year of social work care coordination case management behavioral health healthcare or community-based services experience (Required)
Experience supporting individuals with Intellectual and Developmental Disabilities (ID/DD) and waiver services (Preferred)
Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services (Preferred)
Experience advocating for patients clients or vulnerable populations and assisting with care transitions and service navigation (Preferred)
