Overview:

Sentara Health is hiring a Full Time Care Coordinator for the Roanoke VA area!

Hours: Full Time (40 hrs/wk)

Shift: Day (8am-5pm)

This position requires travel for in person face-to-face assessments in RoanokeVA . Days without assessments scheduled employee can work remotely.

• Must possess a Bachelors degree in Health & Human Services or one of the following related Fields:

  -Community Mental Health Counseling (MH Vocational Pastoral etc.)
  -Counselor Education
  -Early Childhood Development
  -Educational Psychology
  -Gerontology
  -Healthcare Administration
  -Human Development
  -Human Services
  -Marriage and Family Therapy
  -Music Therapy
  -Nursing
  -Pharmacy
  -Psychiatric Rehabilitation
  -Psychology
  -Rehabilitation Counseling
  -Social Work
  -Sociology
  -Special Education
  -Speech Therapy
  -Therapeutic Recreation
  -Vocational Rehabilitation

• No specific certification or licensure requirements

• 1 year of social work care coordination case management behavioral health healthcare or community-based services experience (Required)

• Experience supporting individuals with Intellectual and Developmental Disabilities (ID/DD) and waiver services (Preferred)

• Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services (Preferred)

• Experience advocating for patients clients or vulnerable populations and assisting with care transitions and service navigation (Preferred)
