This program is designed to provide early intervention and community coordination to help keep children out of the juvenile justice We Offer:A Collaborative and Supportive Team AtmosphereCompetitive Compensation and Paid Time Off PackageFamily-Friendly WorkplaceMedical/Dental/Vision Coverage401(k) Retirement Savings PlanContinuing Education OpportunitiesCare Coordinator Job Responsibilities:Completes the intake and assessment of youth and families referred to or self-referred to the ACES Care Coordination that supports youth and family including but not limited tointakeassessmentreferralsand follow-up interactionsin the designated EHR and uploadsadditionalpaperworkas necessaryinto confidentiality and adheres to ethical and legal standardsShares on-call responsibilities with the Program referrals and recommendations based on the identified needs of the youth and family to prevent the youth from entering or progressing further into the juvenile justice youth and family in -up contactsas necessaryto support youth and family Coordinator Qualifications:Bachelorsdegreeand two (2) yearsofexperienceworking withmulti-need youth and familiesormasters in social workor related clinical degree and one (1) year post-graduate experience. Prior related experience may be substituted for one (1) year post-graduate requirement. A general knowledgeof andpractical experience working with families and their needs is preferred.
