The Role:

Role Summary

The Outreach Coordinator serves as the primary point of contact for families referred by Regional Centers guiding them through the initial stages of service enrollment and ensuring a seamless onboarding experience. This role coordinates referrals maintains strong relationships with families and Regional Center staff supports both Agency and Employer of Record (EOR) service lines and partners with Community Partnerships and Operations to ensure timely high-quality service delivery.

Primary Responsibilities

  • Coordinate new referrals from Regional Centers by conducting intake activities responding to family inquiries documenting interactions and ensuring referrals are addressed promptly.
  • Manage referral pipelines by following up with families and providers maintaining accurate records in Salesforce and Annkissam and providing ongoing updates throughout the enrollment process.
  • Support Agency service enrollment by evaluating referrals coordinating with Operations and Community Partnerships and ensuring consumers are routed to the appropriate teams.
  • Guide families through the Employer of Record (EOR) onboarding process by explaining available services collecting provider information assisting with applications and resolving onboarding obstacles.
  • Build and maintain strong relationships with Regional Center staff families and internal partners while tracking personal performance metrics and completing additional administrative duties as assigned.

This is a hybrid position coming into the Concord office up to 3x per week.

Work Schedule: Business hours are 8:00 a.m. to 6:00 p.m. Pacific Time. Depending on the position and business needs employees may be required to work any scheduled shift within these operating hours.

What You Bring to the Table:

Qualifications

  • Fluency in Spanish is required
  • Bachelors degree preferred.
  • 12 years of customer service healthcare operations community outreach or related experience.
  • Experience using CRM or case management systems such as Salesforce preferred.
  • Strong communication organizational and relationship-building skills with the ability to manage multiple priorities in a fast-paced environment.
  • Positive self-motivated professional with strong problem-solving skills and the ability to maintain confidentiality.

The expected California Pay Range for this position:
