LVN (Home Health)

Shifts Available: Full Time

Service Areas for Coverage: Northwest & Northeast

Responsibilities:

• Collaborate with the home healthcare teamto develop and implement individualized care plans.
• Complete home care nursing visits.
• Prepare timely complete and accurate clinical notes.
• Communicate with healthcare providers and other care team members.
• Educate patients and their families about health conditions treatment plans and self-care techniques.
• Adhere to all agency policies procedures and regulatory requirements related to home health care.

Qualifications:

• Valid and current LVN or LPN license in the state of Texas or compact state.
• Minimum of one (1) year of experience in a home health setting.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred but not required)
• Excellent communication and people skills to effectively interact with patients families and healthcare team members.
• Compassionate and empathetic demeanor with a commitment to providing patient-centered care.
