Overview
Behavioral Health Navigator II provides independent patient-centered care coordination to individuals with mental health and/or substance use needs. This role builds on foundational experience and supports moderate to complex patient populations especially those transitioning from emergency or medical floor settings.
Education
Masters Degree in one of the following areas: Social Work Counseling Clinical Mental Health Counseling Marriage and Family Therapy Psychology Addiction Studies Nursing or a closely related field. (Required) OR
Bachelors of Science in Nursing for RN Candidates (Required)
Experience
Five years of healthcare experience delivered in one of the following settings: Acute care long-term care home health hospice skilled nursing facility or community-based program services (Required)

Sentara CarePlex Hospital located in Hampton VA is a 224-bed technologically advanced acute care facility and Certified Primary Stroke Center. Other features include specialized services in heart and vascular urologic care along with advanced diagnostic and surgical capabilities a hospitalist program and one of the states busiest emergency departments.
