We are seeking a compassionate and detail-orientedMedicaid Eligibility Specialistto support older adults and their caregivers through the Medicaid application and eligibility this role you will serve as a trusted resource helping participants navigate Medicaid requirements while ensuring timely and accurate submission of applications re-determinations and supporting documentation.

- Guide participants and families through Medicaid applications approvals and re-determinations.
- Conduct home visits to complete applications and gather required information.
- Collaborate with county agencies participants caregivers and internal teams to ensure eligibility requirements are met.
- Educate families on Medicaid rules and regulations including Long-Term Care Medicaid trusts and spousal impoverishment provisions.
- Monitor and track eligibility status maintaining accurate records and documentation.
- Complete HMO disenrollment paperwork and communicate progress to enrollment and leadership teams.
- Maintain strict confidentiality and compliance with all federal state and organizational requirements.
- High school diploma or equivalent required.
- Comprehensive knowledge of Medicaid programs eligibility requirements and benefits.
- Understanding of intake practices and geriatric assessment principles.
- Knowledge of community resources available to seniors and their families.
- Minimum of one (1) year of experience working with frail elderly or aging populations.
- Strong communication organizational and customer service skills.
- Ability to work independently while building collaborative relationships with participants families and team members.
