We are seeking a compassionate and detail-orientedMedicaid Eligibility Specialistto support older adults and their caregivers through the Medicaid application and eligibility this role you will serve as a trusted resource helping participants navigate Medicaid requirements while ensuring timely and accurate submission of applications re-determinations and supporting documentation.

• Guide participants and families through Medicaid applications approvals and re-determinations.
• Conduct home visits to complete applications and gather required information.
• Collaborate with county agencies participants caregivers and internal teams to ensure eligibility requirements are met.
• Educate families on Medicaid rules and regulations including Long-Term Care Medicaid trusts and spousal impoverishment provisions.
• Monitor and track eligibility status maintaining accurate records and documentation.
• Complete HMO disenrollment paperwork and communicate progress to enrollment and leadership teams.
• Maintain strict confidentiality and compliance with all federal state and organizational requirements.

• High school diploma or equivalent required.
• Comprehensive knowledge of Medicaid programs eligibility requirements and benefits.
• Understanding of intake practices and geriatric assessment principles.
• Knowledge of community resources available to seniors and their families.
• Minimum of one (1) year of experience working with frail elderly or aging populations.
• Strong communication organizational and customer service skills.
• Ability to work independently while building collaborative relationships with participants families and team members.
