POSITION SUMMARY
The Behavioral Health Home Based Clinician is responsible for providing culturally appropriate professional behavioral health (BH) and substance abuse treatment services for the Nottawaseppi Huron Band of the Potawatomi. This position works with a diverse population including children and their family members.
ESSENTIAL FUNCTIONS
- Provides services with specific focus on children and families identified as at risk and specifically provides these services in the family home or in the community.
- Interviews clients family members and other interested parties to assess needs for BH treatment; completes thorough and timely mental health screenings psycho-social assessments including DSM-5 diagnostic impressions and person-centered treatment plans and plan updates.
- Weaves cultural aspects and support throughout all treatment services.
- Provides a clinical assessment regarding the end of services including completion of discharge summaries follow-up care referrals and aftercare planning.
- Performs case management duties and collaboration with social services medical/dental or others as required to meet the clients needs and conducting phone calls referrals follow-up scheduling warm hand offs and/or contacts with clients family.
- Conducts treatment coordination discharge planning aftercare case management and submission of documents and maintains professional relationships with treatment facilities.
- Addresses BH needs of medical/dental patients. This can include stepping into medical/dental visits to offer additional and BH-specific support and guidance consulting with medical and/or dental providers for clients presenting with BH related needs assisting with BH referrals making referrals/connections outside of NHBP services offering brief and solution-focused BH intervention including crisis and substance use disorder interventions and providing screenings.
- Documents and maintains case files in accordance with NHBP policy and procedures.
- Documents correspondence regarding client status upon request which may include court reports or testimony.
- Assists and participates in tribal outreach education events or activities. This may include public speaking.
- Participates in department meetings as directed and/or appropriate including task forces staff meetings team meetings case consultation clinical supervision and other relevant meetings.
- Participates in broad continuous learning and self-assessment activities to address diverse client needs including mental health and substance use domestic violence child abuse/neglect foster care infant mental health trauma medically fragile to perform a multitude of treatment modalities within ones scope of practice.
MINIMUM QUALIFICATION
- Masters degree in social work or closely related field (Professional Counseling or Psychology).
- Licensed in field of practice and be in good standing in the State of Michigan.
- 3 years of experience in the field of social work psychology counseling or behavioral health sciences including familiarity with public assistance and social service systems and special programs for members of tribal organizations.
- Successfully complete and pass a comprehensive background investigation and drug screening in accordance with NHBP standards.
- Significant professional experience organizing performing computer and data management and maintaining electronic files.
- Experience successfully working with diverse groups in community-based team settings.
- Maintain valid drivers license with good driving record and be GSA certifiable. Travel is required.
- Experience working with highly confidential and sensitive information and both preserving and maintaining confidential and sensitive information which includes following HIPAA requirements.
- Work flexible hours when necessary or as directed.
PREFERRED QUALIFICATIONS
- Certified Advanced Alcohol and Drug Counselor or willing to obtain.
- Knowledge of Indian Health Service regulations.
- Experience working with tribal governments or Native American institutions.
- Well-developed knowledge of the historical trauma of indigenous tribal communities and trauma informed care in a general health setting.
