Behavioral Health Consultant ACCESS DPV and ACCESS DMV

Position SummaryThe Behavioral Health Consultant (BHC) is part of a multi-disciplinary primary health care team. BHCs are responsible for providing behavioral health consultation and treatment to children adolescents and adults in order to improve their psychosocial functioning and overall health Job screenings assessments and diagnoses of mental health conditions and functional assessments identifying barriers to improving and maintaining optimal physical crisis situations and apply appropriate in the detection of at risk patients and the development of plans to prevent further psychological or physical the primary care team in developing care management processes to improve patient self-management of chronic brief focused interventions for mental and behavioral health problems and participate in meetings that support ACCESSs integrated healthcare model and promote comprehensive care for our consultation and training to the primary care team to enhance their skill and effectiveness in managing the behavioral aspects of patients; care plans and treating mental health other duties as degree from an accredited school Illinois LCSW/LCPC LMFT or PsyD licensure two (2) years post Masters degree clinical experience working knowledge of evidenced-based mental health and behavioral fluency in Spanish is required for some BHC Conditions/EquipmentHealth Center environmentFrequent trips to the health centersLocal travel between health centers hospitals and home/community visits is required; mode of transportation required if personal vehicle is employed a current drivers license and proof of insurance is requiredRemote working options; must have a suitable and secure remote working environmentAbility to sit for up to 4 hours at a timeAbility to stand for up to 2 hours at a timeComputer/laptopPhone/Fax/Copier/Scanner
