Community of Hope is seeking an Associate Director of Home Visiting to provide leadership and oversight for the Parents as Teachers (PAT) and Healthy Families America (HFA) home visiting programs ensuring compliance with grant requirements and adherence to PAT and HFA model fidelity standards. Additionally the Associate Director of Home Visiting will coordinate all program operations and supervise Family Case Managers to support effective service delivery promote positive parenting practices strengthen parent skills and confidence and prepare children for school success. This position is located at our Conway Health & Resource Center in SW Washington DC.

What Youll Do

  • Provides leadership and management of the home visiting program ensuring full implementation of the Parents as Teachers and Healthy Families America models to fidelity and maintains accreditation.
  • Develops partnerships and manages referrals to behavioral health and other social services.
  • Monitors Child and Family Services Administration referrals and ensures that they are recorded as Unusual Incident Reports. Ensures adequate follow-up on all referrals and reports.
  • Reviews all Family Resiliency and Opportunities for Growth (FROG) scale assessments and intake forms. Assigns cases to home visiting coordinators or home visitors to ensure service delivery within two business days of receipt of assessment.
  • Ensures that all home visiting staff are properly trained to enter data in required data systems (ex. eCW DCRS 2.0 and HV Recruitment Portal FamilyWise and Visit Tracker) within two (2) working days of available information and data.
  • Ensures that all participants are administered required screening tools per grant agreements and model fidelity. Ensures tools are scored and entered into the database within six (6) workdays of the screening being administered.
  • Monitors DCRS Visit Tracker and FamilyWise databases on a daily basis to ensure that contact notes screens and data related to family health is entered appropriately. Provides regular feedback to Family Case Managers on the administrative aspects of their work.
  • Provides support to Home Visiting Coordinators to ensure that the required numbers of home visits are conducted monthly and all documentation is completed accurately and timely.
  • Collaborates with the Senior Director of Maternal & Child Health and the Development team on the preparation of reports to funders according to contract deadlines. Ensures reports and documentation address targets and measures outcomes for activities. Ensures that home visiting staff are aware of targets and outcomes.
  • Completes other tasks as requested by supervisor.
Requirements

Must-Haves

  • B.A. in Human Services related field (social work early childhood education family studies and/or psychology) or equivalent professional experience.
  • At least 2 years of supervisory experience.
  • At least 2 years of management experience in public health and/or human services field.
  • Experience providing or overseeing programs that provide public health case management and education (i.e. reproductive health smoking cessation child development parenting etc.) to at risk populations preferably in the clients home setting.
  • Ability to conduct home visits.
  • Valid drivers license and vehicle required as well as proof of auto insurance.
  • Demonstrated cultural competence in communicating with underserved diverse populations.
  • Strong written and verbal communication skills.
  • Ability to work with computers and electronic health records.
  • Knowledge of community resources or the ability to become knowledgeable.
  • Ability to establish and maintain trusting relationships with families.
  • Strong interpersonal organizational analytical problem solving and cross-cultural sensitivity skills.
  • Ability to work flexible hours including weekends and evenings.
  • Ability to travel between sites required and/or as needed.
