(Epic) Return Staff Policy: Permanent staff must be separated from Providence for a minimum of 3 months to return as a traveler. Travelers can only work for a maximum of 1 calendar year in their assignment. Travelers are then required to take a minimum of a 6-month break before returning for another traveler assignment at the same facility. Facility only accepts Hep B vaccines and will not take titers or declination References Required: 2 References covering 1 year (6 months for CNA s) within the last 3 years during the compliance process and prior to start. If Client does not provide a mobile phone required for the assignment Client agrees to pay Agency monthly for all work-related cell phone use by each Provider up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency.
