- Full-Time Day Shift
- Schedule: MondayFriday 8:00 AM 5:00 PM
- Primary Location: Saint Lukes Plaza
- Opportunity to float to additional sites as needed
As an Advanced Practice Provider (APP) you will provide advanced clinical care to assigned patient populations while serving as a key member of an interdisciplinary team. Responsibilities include:
- Assessing collecting and analyzing patient health data
- Developing implementing and evaluating individualized plans of care
- Coordinating comprehensive patient care and case management services
- Providing expert clinical consultation
- Utilizing prescriptive authority procedures and treatments in accordance with state and federal regulations
- Educating patients families and healthcare staff
- Managing high-risk patient populations
- Supporting quality improvement and research initiatives
- Promoting positive patient outcomes and enhancing care delivery systems
- Call coverage: One week (7 consecutive days) at a time
- Currently call and weekend coverage are provided only at Saint Lukes Plaza
- Call and weekend commitment: Approximately 12 times per quarter
- Holiday commitment: One holiday per year
