Compensation: $100000$140000 annually based on experience up to $9000 in quarterly performance bonuses
Sign-On Bonus: $10000 total $5000 paid after 30 days of employment and $5000 paid after 6 months of employment

This opportunity is ideal for an APP who is passionate about delivering high-quality outpatient care in a collaborative patient-centered environment. We are especially seeking providers who can connect with our diverse patient population with Mandarin-speaking or Spanish-speaking providers.

Youll work alongside physicians care managers behavioral health specialists and support staff to provide comprehensive care focused on prevention chronic disease management and long-term wellness.

• Evaluate diagnose and treat patients in an outpatient primary care setting.
• Conduct wellness visits preventive screenings chronic disease management and acute care visits.
• Develop and implement individualized treatment plans.
• Order and interpret diagnostic tests labs and imaging.

• Educate patients on medications treatment plans nutrition and preventive health.
• Support patients in managing chronic conditions such as diabetes hypertension and asthma.
• Promote continuity of care through proactive follow-up and patient engagement.

• Collaborate closely with physicians and interdisciplinary care teams.
• Coordinate referrals diagnostics and specialty follow-up care.
• Participate in a team-based integrated care model focused on quality outcomes.

• Maintain accurate and timely documentation in the EHR system.
• Ensure compliance with clinical guidelines regulatory standards and quality initiatives.

• Masters or Doctorate degree from an accredited Nurse Practitioner or Physician Assistant program.
• Active Texas license as one of the following:
  • Nurse Practitioner (FNP)
  • Physician Assistant (PA-C)
• Active DEA license or ability to obtain prior to start.
• National certification applicable to role/specialty.
• Strong commitment to patient-centered and culturally competent care.

• Fluent in Mandarin or Spanish.
• Experience in Family Medicine Primary Care Internal Medicine or Pediatrics.
• Experience working in diverse or underserved communities.
• Value-based care or population health experience is a plus.

• Base Salary:$100000$140000 annually based on experience and qualifications.
• Quarterly Performance Bonus: Up to $9000 per quarter based on applicable performance criteria and company programs.
• Sign-On Bonus: $10000 total paid as:
  • $5000 after 30 days of employment
  • $5000 after 6 months of employment
