BAYADA Home Health Care is seeking an experienced Admissions Physical Therapist (PT) for a full-time opportunity with our UCV office providing home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients throughout Monmouth Middlesex Union and Somerset Counties NJ.

As an Admissions Physical Therapist youll play an important role in welcoming new clients to BAYADA and establishing the foundation for a successful home health experience. Youll complete comprehensive physical therapy evaluations assess each clients functional and clinical needs and develop individualized plans of care that help clients safely recover and maintain their independence at home.

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.
• Perform comprehensive physical therapy evaluations including assessments of mobility strength balance gait endurance transfers safety and functional abilities.
• Assess the clients home environment and identify potential safety concerns fall risks and barriers to independence.
• Develop individualized plans of care based on client needs goals functional abilities and physician orders.
• Establish appropriate physical therapy goals and make recommendations for ongoing therapy services.
• Identify additional clinical needs and coordinate appropriate nursing therapy social work and other home health services.
• Educate clients families and caregivers on mobility safety fall prevention therapeutic exercises and the home health plan of care.
• Collaborate with physicians and members of the interdisciplinary care team to coordinate comprehensive client care.
• Ensure timely and accurate completion of OASIS assessments and all required admission documentation.
• Accurately document evaluations clinical findings education and care coordination using BAYADAs electronic medical record (EMR) system.
• Help ensure a smooth transition from referral to admission and ongoing home health services.

• Current and unrestricted New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of recent experience as a Physical Therapist.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment evaluation and care coordination skills.
• Excellent communication interpersonal organizational and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Monmouth Middlesex Union and Somerset Counties.

As an Admissions Physical Therapist youre often the first BAYADA clinician a new client and family meets. Your evaluation and clinical judgment help establish the foundation for the clients home health experience and ensure they receive the services and support needed to safely achieve their goals at home.

Youll have the opportunity to use your physical therapy expertise in a highly autonomous role while still having the support of an experienced interdisciplinary team. Youll focus on one client at a time evaluating their needs within their actual home environment and creating an individualized plan that supports mobility safety independence and quality of life.

Base Pay depending upon qualifications status and subject to negotiation:

$75.00 - $85.00 per point
