Job Summary
General Purpose: The Admission/Discharge Coordinator is responsible for managing the flow of clients entering and exiting the Acute Treatment and Crisis Stabilization Unit. This role ensures timely screening of potential admissions coordination of financial and insurance authorizations facilitation of care coordination activities and development of safe clinically appropriate discharge plans. The coordinator serves as a key liaison between internal clinical teams external referral sources payers and community partners to support continuity of care.
Essential Duties/Responsibilities
- Admission Screening Conduct initial screening of potential admissions for clinical appropriateness safety considerations and level-of-care criteria.
- Authorization Coordination Obtain insurance authorizations verify benefits determine financial eligibility and communicate payer requirements to the treatment team.
- Referral Communication Serve as primary point of contact for hospitals crisis teams law enforcement outpatient providers and community agencies regarding admission status and documentation needs.
- Care Coordination Collaborate with clinical staff case managers and medical providers to ensure seamless transitions throughout the clients stay.
- Discharge Planning Develop safe individualized discharge plans including follow-up appointments referrals safety planning and linkage to community supports.
- Documentation Management Maintain accurate timely records of screenings authorizations financial information and discharge plans in the electronic health record.
- Collaboration Work closely with clinical and medical staff to support rapid admissions and discharges in a fast-paced 24/7 environment.
- Compliance & Standards Ensure adherence to regulatory payer and organizational requirements related to admissions discharges and documentation.
Job Qualifications
Knowledge Skills and Ability:
- Strong understanding of behavioral health crisis stabilization and acute treatment services
- Ability to assess clinical appropriateness for admission using established criteria
- Knowledge of insurance processes authorizations and financial eligibility
- Excellent communication and coordination skills across multidisciplinary team
- Ability to manage multiple priorities in a high-volume fast-paced environment
- Strong organizational and documentation skills
- Ability to maintain professionalism and calm during crisis situations
- Competence in electronic health record systems and documentation standards
Minimum Qualifications
- Bachelors degree in a behavioral health or human services field
- Experience in crisis services acute treatment admissions or care coordination preferred
- Ability to work weekends holidays and flexible hours as needed
Education or Formal Training:
Bachelors degree in behavioral health Social Work Psychology Human Services or related field.
Experience:
Demonstrated experience supporting relational database technology in an operational environment including reporting/query work database administration support and working with sensitive data.

Working Environment:
This job operates in a fast-paced professional office environment and routinely requires the use of standard office equipment such as computers phones photocopiers. The position may require travel to conferences meetings and branch locations on a regular or intermittent basis. Work may involve moderate exposure to unusual elements such as extreme temperatures dirt dust fumes smoke unpleasant odors and/or loud the health center environment there is potential for contact with blood-borne pathogens and communicable diseases as well as potential for contact with dissatisfied or abusive individuals. There could be interaction with persons who are mentally ill disabled elderly and emotionally upset.

This is a sedentary role but can involve standing or sitting for extended periods of time bending at the waist and using hands and fingers to handle and file papers or operate assigned equipment. While performing the duties of this job the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision distance vision color vision and ability to adjust focus. Employee may also have to lift 25 lbs. unassisted.
Conditions of Employment:
- Annual TB federally required drug screening and Influenza vaccination or compliance with policy and procedure.
- Required possession of a valid state drivers license.
- Successful candidate must submit to and pass post-offer drug screen.
