Acute Care Physical Therapist

We are seeking a passionate and dedicated Acute Care Physical Therapist to join our team. The ideal candidate will have a strong passion for patient care and a thirst for continued knowledge and growth.

• Assess patients to develop and initiate individualized treatment plans based on evaluation findings patient needs and goals
• Provide direct patient care using established protocols evidence-based interventions and patient care guidelines
• Document evaluation findings treatment progress and instructions provided to patients and caregivers accurately and timely
• Provide appropriate oversight of patient care performed by physical therapist assistants and rehabilitation technicians
• Complete a written plan of care establish measurable goals based on evaluation findings and develop a comprehensive treatment program to achieve those goals
• Implement individualized physical therapy treatment programs to improve mobility function strength and overall independence
• Communicate effectively with patients caregivers and professional colleagues including physicians nursing staff case management and insurance providers to coordinate care and support optimal patient outcomes
• Complete required documentation including progress notes treatment summaries and discharge recommendations to support continuity of care

Qualifications :

• Graduate from a CAPTE-accredited Physical Therapy program
• Current state of Georgia Physical Therapy license CPR certification
• Exercise sports rehab and manual-based skills
• Outgoing and energetic personality
• New graduates are welcome!
