Position Summary

As the 1915(i) Program Coordinator you will play a vital role in overseeing and coordinating services under the 1915(i) Medicaid program. This position requires a skilled professional with at least 3 years of experience in behavioral health or community-based services. Youll work closely with individuals and families to ensure they receive the support and resources they need to achieve long-term success.

Key Responsibilities

In this role you will:
- Coordinate and oversee the implementation of 1915(i) program services.
- Collaborate with individuals families and community partners to develop and monitor service plans.
- Ensure compliance with Medicaid guidelines and program requirements.
- Provide case management and resource navigation to connect clients with appropriate services.
- Support individuals in achieving stability independence and personal growth.
- Maintain accurate documentation and reporting for program activities.
- Work closely with the Connected Crew Health team to address client needs and improve service delivery.

Required Skills and Qualifications

To succeed in this role youll need:
- A minimum of 3 years of experience in behavioral health or community-based services.
- Strong knowledge of Medicaid programs specifically 1915(i) services.
- Excellent organizational and coordination skills.
- The ability to build relationships with clients families and community partners.
- A commitment to promoting stability independence and personal growth for individuals and families.
- Strong written and verbal communication skills.
