Are you an experienced Nurse Practitioner passionate about delivering exceptional care to older people Join our Residential Aged Care In-Reach (RAC-I) team and be part of an innovative service that is transforming the delivery of acute care across the Hunter region.
In this rewarding role you will provide advanced clinical care within residential aged care facilities delivering timely high-quality treatment that supports early intervention reduces avoidable hospital presentations and helps residents remain safely in their familiar environment.

About the Role

As a Nurse Practitioner you will work collaboratively with Geriatricians Medical Officers Nurses Allied Health Professionals residents families and carers to deliver expert clinical assessment diagnosis treatment and care coordination within your advanced scope of practice.
You will play a key role in providing resident-centred acute care research education and supporting clinical excellence and contributing to service development within an innovative and growing model of care.

About You

You are a motivated and dynamic clinician with:

  • Strong clinical expertise in aged care and/or acute care.
  • Excellent critical thinking and problem-solving skills.
  • The ability to work both autonomously and within a multidisciplinary team.
  • Exceptional communication and stakeholder engagement skills.
  • A commitment to clinical excellence and ongoing professional development.

Essential Requirements

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Current Nurse Practitioner endorsement or substantial progress towards Nurse Practitioner endorsement.
  • A minimum of three years full-time equivalent experience in an advanced practice nursing role.
  • A current unrestricted NSW/Australian Driver Licence
