Job Summary
Responsibilities
Conduct community outreach and awareness programmes on social issues such as substance abuse domestic violence and HIV.
Liaise with schools healthcare facilities law enforcement and other agencies to coordinate holistic client support.
Facilitate access to social services including grants housing healthcare and other support programmes.
Supervise social auxiliary workers volunteers and community caregivers providing guidance and support.
Conduct comprehensive assessments of clients social emotional and economic circumstances to identify needs and risks.
Develop and implement individualised intervention and care plans in collaboration with clients families and other professionals.
