What youwill bedoing

• Conducting service co-ordinations for clients with a range of physicalintellectualand sensory needs

• Coordinating tailored support packages within funding requirements and individual goals

• Building quality relationships with clients whānau providers and teams across the organisation

• Maintaininghigh quality documentation using the Socrates IT system

• Working remotely from anywhere in Aotearoa-Auckland Wellington ChristchurchDunedinor the regions

What youwill bring

• Proven NASC experience within the health or disability sector

• Strong working knowledge of Socrates and funding allocation requirements

• Experience coordinating natural and formal supports for people with diverse needs

• Excellent communication skills and the ability to connect across diverse cultural backgrounds

• High attention to detail strong organisational skills and confidence working independently
