Responsibilities:
- Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
- Admissions of patient for services and development of the initial patient/family Plan of Care
- Uses health assessment data to determine nursing diagnosis
- Coordination of services with the patient family and hospice interdisciplinary team members
Requirements:
- Valid Registered Nurse (RN) license
- RN Diploma Associates or B.S. degree in nursing
- Minimum 1 year of clinical experience home care or hospice preferred
- Excellent observation problem solving and communication skills
- Compassion and integrity
