LMSW Home Health

Seasoned Recruitment is currently seeking compassionate and experienced Licensed Masters Social Workers (LMSWs) to join our Home Health team. As an LMSW in Home Health you will play an essential role in providing comprehensive social work services to patients in the comfort of their own homes.

In this role you will be responsible for conducting psychosocial assessments developing care plans providing counseling and support to patients and their families and coordinating community resources. You will work closely with a multidisciplinary team to address the emotional social and environmental factors that impact patient care and wellbeing.

Requirements:

To be considered for this position you must meet the following requirements:

• Hold a valid and unrestricted Licensed Masters Social Worker (LMSW) license
• Have a minimum of 2 year of experience as an LMSW
• Possess strong knowledge and understanding of social work principles and practices
• Demonstrate excellent communication and interpersonal skills
• Be selfmotivated organized and able to work independently
• Have a reliable means of transportation and a valid drivers license
• Be willing to travel within the designated service area
