Job Description

Our client has been providing supportive housing and shelter for over 35 years with a focus on the chronically homeless people with complex mental health and addiction issues and seniors.

Our clients operate from a harm reduction and clientcentric approach as is looking for a dedicated Intensive Case Management Worker.

The position:

  • Support clients in their housing by assessing and supporting their needs
  • Facilitate tenant movement from shelters to secure longterm permanent housing
  • Assist tenants in maintaining housing
  • Work with site staff to develop outreach and needs assessment criteria
  • Knowledge of the RTA and Hostels Guidelines and Procedures
  • Knowledge of current housing and support agency providers appropriate to our tenants and residents
  • Ability to understand and assess the suitability of potential landlords and/or residences
  • Identify current issues determine support necessary refer and support clients in preparation to entrylevel/transitional housing and with the referring worker support move to site
  • Confer with the frontline support staff at the site to transfer information from the case file to the staff team
  • Utilize case management as a tool to support residents in meeting their goals
  • Provide support including work readiness basic life management skills anger management addiction counseling assertive communication setting goals giving and receiving feedback selfesteem conflict resolution problemsolving and managing change
  • Provide support to clients who are facing issues of poverty mental health addictions abuse social isolation and more
  • Apply knowledge of group processes and dynamics.


Requirements

Your skills and experience:

  • You will have a minimum of 12 years of experience working with people who are homeless underhoused or hard to house
  • Posses a postsecondary degree or diploma in social work or an equivalent combination of education training and experience.
  • Knowledge of tenant population diversity issues antidiscrimination practices and systemic reasons for homelessness
  • Knowledge of relevant laws standards and guidelines resources and services available in the community
