LINQM is looking for someone that is interested in a long term career with a Non Profit as a Case Manager.
This position provides clients with the services needed in order to obtain and maintain income and housing.

Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Provides ongoing intensive support to clients which can include assessing evaluating and coordinating services crisis intervention and applications for benefits & referrals to community resources.
Communicates and advocates to landlords to support clients obtaining and maintaining housing placement.
Prepares presents and documents client cases.
Participates as a member of a multidisciplinary team that prepares and presents client case presentations.
Assesses clients for employment social security and disability insurance eligibility gathers records and schedules treatment.
Job Requirements
Bachelors Degree (some grants require this level of education) in a Social Services field or equivalent experience.
At least 6 months of experience working with underserved populations in a social service setting.
Basic user of MS Office.
Be at least 21 years of age with a minimum of 3 years of driving experience; possesses a valid California drivers license; and has no Class 1 or more than two Class 2 violations within the past 36 months.
Participate in an annual Tuberculosis screening and/or other screening
