Home Care Physical Therapist- Part Time (Passaic County)

We are looking for a motivated and passionate part time (with benefits!) Physical Therapist to provide in-home care. • Job Type: Part time w/ benefits 20-visits per week
• Location: Passaic County (In & around Paterson)
• Schedule: Flexible!

Responsibilities:

• Accept and treat patients within your designated geographic area 
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards agency policies and the prescribed plan of care
• Educate family members home health aides and other caregivers on proper therapy techniques
• Plan and carry out occupational therapy programs to maintain improve or restore physical function relieve pain and prevent occupational dysfunction in patients.
• Perform and document an initial exam evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

Qualifications :

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jersey or eligible.
• Preferred Certifications: First Aid AED & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients homes within the assigned geographic territory

Additional Information :

The anticipated base salary range for this position is $65000-$75000. Salary is based on various factors including relevant experience knowledge skills other job-related qualifications and geography. Medical dental vision 401(k) paid time off and other benefits are also available subject to the terms of the Companys plan.
