Your Opportunity
Targeted Care Packages (TCP) support children and young people aged 019 years to transition from residential or homebased care into stable sustainable care arrangements. These placements may include reunification kinship care foster care or independent/semiindependent living. TCP provides intensive coordinated case management to ensure decisions are guided by the best interests of the child or young person.
As a Senior Key Worker you will play a critical leadership and practice role within the Targeted Care Packages program. You will provide contracted case management support complex transitions and offer formal and informal supervision to a small team of Key Workers and Support Workers.
You will work closely with children young people families carers communities and statutory partners to build safe stable placements and positive life outcomes while supporting a collaborative reflective and traumainformed team culture
- This role requires flexibility to work across locations and occasional afterhours/oncall work as required
About You
We are looking for a collaborative and empathetic practitioner you communicate clearly build trusted relationships and advocate confidently for children young people and families.
You will:
- Lead with cultural humility adaptability and calm judgement balancing complex priorities.
- Support and uplifting others through reflective strengthsbased practice.
- Have a demonstrated ability to achieve positive outcomes for young people through a strengths-based outcomes-focused case management approach is also essential.
- Have a minimum Bachelor qualification in Community Service Social Work or a related discipline.
