Responsibilities:
- Accept and treat patients within your designated geographic area
- Evaluate patients needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician patient and family
- Plan prepare and implement customized therapy programs to restore function and promote independence
- Instruct family members PTAs Home Health Aides and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients families and interdisciplinary team members especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
Qualifications :
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the state of New York or eligible.
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
- Ability to travel to patients homes within the assigned geographic territory
