• 03:00 PM 11:30 PM

Job Summary

Navitas Healthcare LLC is seeking an experienced and compassionate Licensed Practical Nurse (LPN) for a 13-week contract opportunity within a long-term care and skilled nursing facility setting in Falmouth Massachusetts.

The ideal candidate will have strong long-term care experience and the ability to provide high-quality resident-centered care in a fast-paced healthcare environment. This role involves medication administration direct resident care clinical documentation and supervising support staff while collaborating with interdisciplinary healthcare teams.

Key Responsibilities

• Provide direct nursing care in accordance with physician orders and individualized care plans
• Administer medications treatments and therapeutic procedures safely and accurately
• Supervise nursing assistants and support staff during assigned shifts
• Monitor resident conditions and report changes to physicians and nursing leadership
• Maintain accurate documentation including nursing notes physician orders and resident records
• Assist with admissions transfers and discharges
• Perform treatments including wound care catheter care suctioning tube feedings dressing changes and specimen collection
• Monitor and document vital signs and resident assessments
• Educate residents and families regarding care plans and treatment protocols
• Participate in interdisciplinary care planning and support quality care initiatives
• Ensure compliance with facility policies patient safety standards and resident rights
• Maintain confidentiality of resident health information

Requirements

• Active Massachusetts LPN License required
• Graduate of an accredited Nursing Program or approved LPN Program required
• Previous long-term care or skilled nursing experience preferred
• Strong leadership communication and documentation skills
• Ability to work effectively in a fast-paced healthcare environment
• Ability to meet physical job requirements including lifting up to 50 pounds
• English proficiency required (Read Write Speak)

Education

• Nursing Degree from an accredited college/university OR approved LPN program required

Certifications

• Massachusetts LPN License required
• BLS / CPR / AED Certification required
