Job Summary
Navitas Healthcare LLC is seeking an experienced and compassionate Licensed Practical Nurse (LPN) for a 13-week contract opportunity within a long-term care and skilled nursing facility setting in Falmouth Massachusetts.
The ideal candidate will have strong long-term care experience and the ability to provide high-quality resident-centered care in a fast-paced healthcare environment. This role involves medication administration direct resident care clinical documentation and supervising support staff while collaborating with interdisciplinary healthcare teams.
Key Responsibilities
- Provide direct nursing care in accordance with physician orders and individualized care plans
- Administer medications treatments and therapeutic procedures safely and accurately
- Supervise nursing assistants and support staff during assigned shifts
- Monitor resident conditions and report changes to physicians and nursing leadership
- Maintain accurate documentation including nursing notes physician orders and resident records
- Assist with admissions transfers and discharges
- Perform treatments including wound care catheter care suctioning tube feedings dressing changes and specimen collection
- Monitor and document vital signs and resident assessments
- Educate residents and families regarding care plans and treatment protocols
- Participate in interdisciplinary care planning and support quality care initiatives
- Ensure compliance with facility policies patient safety standards and resident rights
- Maintain confidentiality of resident health information
Requirements
- Active Massachusetts LPN License required
- Graduate of an accredited Nursing Program or approved LPN Program required
- Previous long-term care or skilled nursing experience preferred
- Strong leadership communication and documentation skills
- Ability to work effectively in a fast-paced healthcare environment
- Ability to meet physical job requirements including lifting up to 50 pounds
- English proficiency required (Read Write Speak)
Education
- Nursing Degree from an accredited college/university OR approved LPN program required
Certifications
- Massachusetts LPN License required
- BLS / CPR / AED Certification required
