Job Title: Registered Nurse (RN) Home Health

Contract Duration: 13 or 26 Weeks
Shift: Days Timing: 8:00 AM 5:00 PM 40 Hours/Week
Position Overview:
Navitas Healthcare LLC is seeking Registered Nurse (RN) Home Health for an exciting Travel or Local job in Boise ID.
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
- Conduct comprehensive patient assessments and ongoing evaluations
- Develop implement and update patient care plans based on clinical findings and patient needs
- Complete accurate OASIS documentation and maintain timely electronic medical records
- Administer medications wound care treatments and other nursing interventions as prescribed
- Educate patients and family members regarding medications disease management treatment plans and home care instructions
- Coordinate care with physicians therapists case managers and interdisciplinary healthcare teams
- Monitor patient progress and report changes in condition to appropriate providers
- Ensure compliance with home health regulations infection control standards and agency policies
- Maintain productivity standards and manage daily scheduling efficiently
Required Qualifications
- Active Registered Nurse (RN) license in the applicable state
- Current BLS/CPR certification required
- Minimum of 1 year of recent Home Health RN experience required
- OASIS documentation and billing experience required
- Experience with Homecare Homebase software required
- Strong clinical assessment and care coordination skills
- Excellent communication time management and documentation abilities
- Ability to work independently and manage a patient caseload effectively
- Reliable transportation and valid drivers license may be required for patient visits
