Shift: Days Timing: 8:00 AM 5:00 PM 40 Hours/Week

Position Overview:

Navitas Healthcare LLC is seeking Registered Nurse (RN) Home Health for an exciting Travel or Local job in Boise ID.

Key Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
• Conduct comprehensive patient assessments and ongoing evaluations
• Develop implement and update patient care plans based on clinical findings and patient needs
• Complete accurate OASIS documentation and maintain timely electronic medical records
• Administer medications wound care treatments and other nursing interventions as prescribed
• Educate patients and family members regarding medications disease management treatment plans and home care instructions
• Coordinate care with physicians therapists case managers and interdisciplinary healthcare teams
• Monitor patient progress and report changes in condition to appropriate providers
• Ensure compliance with home health regulations infection control standards and agency policies
• Maintain productivity standards and manage daily scheduling efficiently

Required Qualifications

• Active Registered Nurse (RN) license in the applicable state
• Current BLS/CPR certification required
• Minimum of 1 year of recent Home Health RN experience required
• OASIS documentation and billing experience required
• Experience with Homecare Homebase software required
• Strong clinical assessment and care coordination skills
• Excellent communication time management and documentation abilities
• Ability to work independently and manage a patient caseload effectively
• Reliable transportation and valid drivers license may be required for patient visits
