Pay: $28.10 - $33.30 based on experience.

Schedule: Monday - Friday with occasional weekends

On Call Rotation is required.

About the job:

The LPN is responsible for the effective management and provision participant medications provided via the PACE medication department. Administers medications implements primary care orders and evaluates participant needs physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regards to plan of care and medications. Communicates with participants primary care givers physician clinic staff and interdisciplinary team.

ALL required qualifications will be verified before start date.

  • Have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • A minimum of one-year experience in working with the frail or elderly population.
  • Current Kentucky Practical Nurses License (L.P.N.) Cardio Pulmonary Resuscitation (CPR) certification required.
  • Have a valid drivers license proof of insurance and have means of transportation.

Essentials:

  • Responsible for processing of medication orders for all participants through direct input or processing of medication orders input by staff provider ensuring that a complete accurate and legal prescription is received by the dispensing pharmacy.
  • Reviews notes from outside providers and identifies and processes medication orders with approval of PACE provider.
  • Responsible for coordination to maintain participant medication schedules (maintenance of cycle system) through implementation of new medication orders mid-cycle medication changes discontinuations and reconciliations through transition process as prescribed by policy.
  • Responsible for medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge) for all participants in accordance with organization procedures and facilitates any changes required.
  • Responsible for communication with and between PACE providers community-based providers and the pharmacy in regard to medication orders clarifications and pharmacy recommendations.
  • Responsible for coordinating with local pharmacies to obtain urgent medications that are not available through on-site dispensing. Coordination involves transmission of medication order demographics and insurance billing information.
  • Responsible for coordinating and/or facilitating on-site medication dispenses through the system provided through the pharmacy. Also responsible for periodic inventory and maintenance of on-site medication stock.
  • Responsible for assessment and entry of all medication orders for new enrollments setting up pharmacy cycle delivery system assistive systems if needed linking diagnosis information to medications (through H&P) and verifying all information in writing with participants primary care provider. Also responsible for coordinating medications for disenrolling participants to provide needed medications for a period of twenty-eight (28) days after leaving program.
  • In coordination with medication technicians responsible for filling and/or quality control of alternative medication delivery systems (MRB Med-E-Lert etc.) in accordance with medication orders and organizational policies/procedures.
  • Responsible for coordinating delivery of medications from pharmacy to participant either through direct carrier shipping or delivery through or from PACE center. Adheres to organization policies in maintenance of documentation of delivery.
