Job Summary

Job Title: Transition Specialist

Assignment Duration: 6 months

Location: Northeast Ohio (Cleveland Akron Canton OH)

Shift: Mon-Fri 8AM-5PM EST

Additional 2 weeks of training 1on1 CAMERA ON NO TIME OFF DURING TRAINING

PR: XXXX0/hr.

Work Setting:
  • Visiting members in nursing facilities and dropping off documents at local Social Security Administration offices
  • Approximately 2 trips per week; remaining work remote documentation
  • Mileage reimbursed
  • Travel not anticipated over 2 hours depending on location

Position Summary:

The Transition Specialist will support members in Northeast Ohio with SSDI applications by conducting in-person visits in skilled nursing facilities gathering required information obtaining signatures collecting medical records and submitting documentation to Social Security Administration offices. This role requires strong organizational skills detailed documentation abilities and the ability to work independently in a highly autonomous environment.

Key Responsibilities:

  • Review SNF census and navigate internal systems
  • Gather medical records according to SOAR SSA procedures
  • Meet with members and family members to collect required information
  • Conduct interviews and obtain signatures for SSDI applications
  • Communicate with members experiencing speech or language difficulties
  • Coordinate with SNF staff and social workers
  • Submit required documentation to SSA offices
  • Track and follow up on application status and feedback
  • Maintain detailed documentation and reporting
  • Work independently while managing multiple tasks and deadlines

Requirements:

  • Bachelor s degree or equivalent experience
  • 2 4 years of related experience in social work nursing skilled long-term care facilities member-facing roles or documentation-heavy roles.
  • Strong written and verbal communication skills
  • Ability to work independently and troubleshoot issues
  • Valid driver s license required.
  • SOAR certification preferred or must obtain within the two weeks of the role.

Disqualifiers:

  • Inability to obtain SOAR certification.
  • Short-term job history unless clearly identified as contract roles.
  • Lack of commitment to long-term role.

Top Required Skills:

  • ADL (Activities of Daily Living) knowledge and EHR clinical chart interpretation.
  • Detailed documentation and report writing with independent problem-solving ability.
  • Experience communicating with diverse populations including individuals with speech and language difficulties.
