- Coordinate and oversee the delivery of personal care services in alignment with client plans of care
- Maintain accurate and up-to-date records for client services tasks schedules and caregiver assignments
- Build maintain and update caregiver schedules to ensure appropriate coverage and continuity of care
- Review and maintain contract-specific documentation and ensure authorizations align with scheduled services
- Educate clients on Elara Caring policies and applicable state and federal regulations
- Monitor daily service delivery to ensure adequate caregiver availability and timely staffing of open shifts
- Provide guidance support coaching and direction to caregivers to promote quality care and performance improvement
- Ensure caregiver compliance with training certifications in-services health requirements and regulatory standards
- Conduct caregiver orientations as required and partner with Human Resources on disciplinary actions when necessary
- Review reports to ensure accuracy in billing payroll utilization and service delivery
- Collaborate with payroll and timekeeping teams to resolve timesheet discrepancies
- Review utilization reports identify trends and address inconsistencies on a daily basis
- Review and submit incident reports and workers compensation documentation as required
- Communicate professionally and effectively with caregivers clients case managers and internal leadership
- Consult with leadership regarding employee performance concerns client issues or operational risks
- Maintain confidentiality and compliance with HIPAA and all regulatory licensure and contract requirements
- Participate in branch growth initiatives operational planning and professional or community engagement activities
- Represent Elara Caring in a positive professional manner at all times
- Perform additional duties and special projects as assigned
- High School Diploma or GED required
- Associates or Bachelors degree preferred
- Two (2) years of experience in a fast-paced office or care coordination environment preferred
- Experience working with elderly or disabled populations preferred
- Working knowledge of Medicare/Medicaid and home care benefits preferred
- Strong computer proficiency and documentation skills
- Excellent organizational and multitasking abilities
- Professional verbal and written communication skills
- Team-oriented with leadership and motivational abilities
- Ability to work effectively in a fast-paced environment
- Knowledge of the healthcare industry preferred
