Your role and responsibilities
In this role youll be at the heart of shaping and improving how care services are delivered working closely with colleagues providers and partners to make a real difference to peoples lives.
- Work closely with hospital discharge teams social workers contracts and Home First teams to enable safe timely discharges and reduce delays across the wider health and social care system
- Support the effective planning and rollout of new systems that improve service flow and operational efficiency
- Support effective care planning by monitoring vacancies capacity and improving access to residential and home care information reducing delays and enabling informed choice
- Review outstanding care requests and use data feedback and performance insights to identify trends support the development of more efficient processes and drive continuous improvement and informed decision-making
- Provide flexible support including occasional weekend working to meet service demand
You will need
Qualifications and Professional memberships
- To be educated to a good standard with NVQ Level 4 in Business Administration (or equivalent qualification) or relevant demonstrable experience
- Strong literacy and numeracy skills
- Knowledge of health and social care settings (desirable but not essential)
- Experience of monitoring reviewing and improving processes and procedures
- Accurate recording reporting and documentation skills
- Proficient in Microsoft Office and standard digital systems
- Ability to provide clear information advice and excellent customer service
- Strong communication negotiation and influencing skills with the ability to engage others clearly empathetically and with impact
- Communicates openly and honestly sharing ideas and information in a clear concise and tactful way
- Remains resilient and effective under pressure adapting positively to new systems processes and environments
- Demonstrates strong organisational skills with the ability to plan prioritise and meet deadlines
- Maintains confidentiality and adheres to organisational policies procedures and values
- Consistently demonstrates ASPIRE values and behaviours in everyday practice
- Flexibility and willingness to travel to locations across the County including clinical environments

The team
The Brokerage Team plays a key role in sourcing care services for individuals who have been assessed as having eligible needs. Acting as a central point of contact for social care placements in Suffolk the team works across Adult and Community Services maintaining close and regular collaboration with care providers health and social care colleagues. A core focus of the team is to reduce delays by ensuring people receive timely access to the care and support they need.
The team is dedicated to promoting preventative outcomes safeguarding vulnerable individuals and ensuring that support is arranged as quickly and effectively as possible. This is delivered in line with key directorate operating models including Supporting Lives Connecting Communities and Signs of Safety.
