Responsibilities

• Navigates patient through the continuum of care.
• Educates patients (and possibly family members) on topics such as the nature and status of patients condition and care deliver goals based on patient specific care plan.
• Serves as liaison for patients/families providers nursing and other appropriate staff.
• Expedites flow of patients through clinic appointments; works with support staff to coordinate and schedule patients appropriately
• Proactively assesses the needs of the patient prior to arrival through review of clinic provider treatment and appointment schedules.
• Provides a visible and specific channel through which patients can seek solutions to problems concerns and unmet needs.
• Interacts regularly or as needed with the multidisciplinary team clinic staff clinical support services social workers dietary patient resource services financial counseling and other hospital services to ensure integrated care.
• Participates in and/or leads quality improvement processes to enhance the operations of the program and overall patient satisfaction.
• Develops improves and coordinates a system which organizes the flow of patients from referral through treatment including evaluation and education and support.
