Enhances the patient experience by providing a seamless navigation process from diagnosis through end of care. Responsible for collaborating with physicians and members of the multidisciplinary teams to triage coordinate and consistently manage patient care by assuming the role of resource person for the patient and the multidisciplinary team. Works with patients and families to answer questions provide emotional support and solve issues concerning the cycle of care.Education & Experience
Education and Experience Required:
Two (2) years of work experience as a Registered Nurse.
Certifications Licenses or Registration required:
Valid RN License
Preferred Qualifications:
Specialty or case management experience

Knowledge of clinical nursing care procedures and protocols. Verbal and written communication skills. Interpersonal skills. Knowledge and understanding of computer systems tools and programs. Ability to work in a team environment.
Responsibilities
- Navigates patient through the continuum of care.
- Educates patients (and possibly family members) on topics such as the nature and status of patients condition and care deliver goals based on patient specific care plan.
- Serves as liaison for patients/families providers nursing and other appropriate staff.
- Expedites flow of patients through clinic appointments; works with support staff to coordinate and schedule patients appropriately
- Proactively assesses the needs of the patient prior to arrival through review of clinic provider treatment and appointment schedules.
- Provides a visible and specific channel through which patients can seek solutions to problems concerns and unmet needs.
- Interacts regularly or as needed with the multidisciplinary team clinic staff clinical support services social workers dietary patient resource services financial counseling and other hospital services to ensure integrated care.
- Participates in and/or leads quality improvement processes to enhance the operations of the program and overall patient satisfaction.
- Develops improves and coordinates a system which organizes the flow of patients from referral through treatment including evaluation and education and support.
