Work Schedule:
100% FTE working 7:00 am 5:00 pm Seven days on and seven days off. This will include every other weekend and will include holidays as scheduled. This will be a hybrid position with most clinical hours to be completed virtually after orientation.

As an Advanced Practice Provider youll play an integral role in our inpatient care teams by providing high-quality cost-effective care to patients.
We are seeking a Nurse Practitioner or Physician Assistant to:
- Independently care for the hospitalized patients in the UW Home-based Hospital Care Program.
- Work with patients with acute and chronic disease.
- Perform assessment diagnosis treatment management prevention care coordination including multidisciplinary care rounds and management of acute and chronic conditions.

Education:
Minimum
- If Nurse Practitioner: Masters or Doctorates degree from accredited Nursing Program.
- If Physician Assistant: Bachelors or Masters degree from accredited PA Program.
Licenses and Certifications:
Minimum State of Wisconsin NP or PA licensure
If Nurse Practitioner*:
- Active RN and APRN certifications and licenses including prescriptive authority in state(s) of practice AND
- Active board certification as a NP in area of clinical practice. These may include the following: Adult/Gerontology Acute Care or Adult/Gerontology Primary Care with clinical experience in the hospital or previous ICU experience as Nurse or Nurse Practitioner.
If Physician Assistant*:
- Active PA license including prescriptive authority in state(s) of practice AND
- Active PA board certification
All Nurse Practitioner and Physician Assistant applicants must have:
- Active DEA license or ability to apply for such license prior to or upon hire.
- Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.
