Role Overview:
The Community Care Coordinator is part of a multidisciplinary clinical team who serves as the first clinical contact within the clinic as well as the bridge between clinical services and essential community resources. The role includes conducting phone and face screenings and intakes on-site and off-site for veterans and their families as well as facilitating direct referrals for housing employment healthcare legal education peer-support and all other essential community-based needs for our clients. Additionally the Community Care Coordinator manages referral partnerships and relationship building through community outreach and engagement post-referral to ensure the provision of quality collaboration with clinic leadership and case managers the role assists the clinical team in understanding resources and needs within the clinic community.
This role is expected to spend approximately 50% of their time in the community conducting outreach and in community facing activities such as attending events sharing our mission and building community relationships across our region.
Masters level Care Coordinators can also provide direct therapy services to a small case load if they choose.
As a Member of the Military Family Clinic You Will Have the Opportunity to:
- Training: Have access to a wide range of training to support high-quality clinical practice with a specific focus on evidence-based practices such as CBT PE CPT and CBT-I as well as population-specific training such as Military Cultural Competence and training focused on treating families couples and children/adolescents.
- Evidence Based Practices: Provide evidence-based treatment which is a systematic process of decision-making to include the best available research to guide treatment decisions and clinical practices in the context of patient characteristics.
- Participate in a multi-disciplinary team: Our clinic includes varied mental health specialties and who work together to meet the needs of our clients.
- Flexible schedules: We offer a variety of work schedules including hybrid remote/in clinic schedules. Our leadership works with each teammate to identify a schedule that meets both their needs and the needs of those we serve.
Key Responsibilities:
Responsibilities include but are not limited to:
- Conduct initial intakes with veterans and/or family members. Coordinate case assignment with Cohen Clinic multi-disciplinary team. Document detailed intakes on the electronic health records (EHR) system; maintain comprehensive client referral tracking system. Prepare reports as requested by Clinic Director.
- Provide services via telehealth platform as required.
- Meet regularly with the Clinic Director and/or Lead Clinician to coordinate case assignments client activities at satellite locations report referral to admissions conversion rate; Provide on-going program evaluation and recommendations to the Clinic Director for continuous growth and quality.
- Engage in community outreach and relationship building. Provide presentations at various community groups within our region. Promote the success and reputation of the Cohen Clinic and its programs.
- Knowledge of local community resources and provide warm hand-offs for veterans and families needing external referrals.
- Provides comprehensive case management to veterans and their families who are seeking mental health services.
- Develops network of trusted contact within organizations in which to refer and coordinate care.
- Follows care follow-up procedures to ensure veterans and/or their family receive the services they were referred to and do not have any follow-up needs.
- Assists veterans and their family members navigate service providers across multiple systems of care.

- Ability to think analytically and evaluate the impact of case management recommendations.
- Ability to set work priorities and to evaluate and create solutions to work related problems.
- Ability to be self-directed and work independently while keeping supervisor informed as well as part of a team.
What Were Looking For:
- Masters degree in social services field. Active full or associate licensure. Maintain license and/or certifications as required by state regulations. Stay current in the field of mental health counseling case management and other related social services.
- 3 years post masters intake coordination and/or case management experience preferred.
- Knowledge of/experience working with local social service public health and VSO organizations and resources. Experience working with a military or veteran population is preferred.
- Must demonstrate leadership capacity and ability to foster team building and a supportive flexible environment.
- Working knowledge of Outlook Word and Excel; PowerPoint and Access a plus.
- Knowledge of case management services and community resources.
- Knowledge of/experience working with local social service public health and VSO organizations and resources.
- Experience working with a military or veteran population is preferred.
- Must demonstrate leadership capacity and ability to foster team building and a supportive flexible environment.
- Working knowledge of Outlook Word and Excel; PowerPoint and Access a plus.
- Knowledge of mental illness and mental health diagnosis. Knowledge of various treatment modalities and methods.
- Clinic Hours of Operation: 8:00am - 7pm Mon-Thurs and 8:00 - 3pm on Fridays. Employees should expect to work some evenings and/or weekends. Flexible schedules are available.
