Key Responsibilities
Care Coordination & Client Support
- Serve as the primary point of contact for clients families and care team members
- Develop and monitor individualized care plans to ensure client needs are being met
- Address client concerns and escalate complex cases to the appropriate team members
Scheduling & Operations
- Coordinate and manage caregiver schedules to ensure consistent coverage
- Track and follow up on authorizations plan changes and service updates
- Maintain accurate and up-to-date client records and documentation
Communication & Collaboration
- Liaise between clients families caregivers and internal teams to ensure seamless care delivery
- Provide timely updates to all parties on care plan changes and service status
- Support intake and onboarding of new clients as needed
Qualifications
- Experience in a care coordination case management or homecare role
- Strong verbal and written communication skills
- Highly organized with excellent time management and follow-up abilities
- Compassionate professional and committed to client confidentiality
- Comfortable working in a fast-paced in-office environment
