Job Summary
Human Touch Home Health is seeking a licensed Physical Therapist (Per Diem) to provide home health services throughout Fairfax County Loudoun County and Prince William County Virginia.

What Youll Do
- Complete evaluations treatment plans reassessments and discharge planning
- Provide skilled physical therapy services in patients homes
- Coordinate care with physicians and the interdisciplinary team
- Manage your own schedule and service area

Requirements
- Active Virginia Physical Therapist license
- Valid drivers license and reliable transportation
- Strong documentation and communication skills
- Home health experience preferred
