Responsibilities

• Provide hands-on personal care to clients such as assisting with bathing dressing grooming and mobility ensuring their comfort and dignity at all times.
• Help clients with daily living activities including meal preparation feeding light housekeeping and medication reminders to promote their independence and well-being.
• Monitor clients health and report any changes or concerns promptly to nursing staff or family members ensuring safety and proper care coordination.
• Offer companionship and emotional support to foster a positive and encouraging environment helping clients stay engaged and mentally stimulated.
• Maintain accurate and detailed records of care provided and client progress to ensure continuity and quality of care.

Requirements

• Current certification as a Home Health Aide is required.
• Must have worked with a home care agency within the last 2 years.

• Reliable transportation to commute between Southold and Riverhead locations ensuring timely attendance and flexibility.
