Palliative Care Nurse Practitioner
Summary of Responsibilities:
The Palliative Care Nurse Practitioner is responsible for the determination of appropriateness of patients seeking Palliative Care services and providing specialized care and support to patients receiving Palliative Care services. This includes collaboration with Compass Medical Director(s) the patients attending physicians and specialists for the purpose of coordination of care to meet the medical needs of the patient while focusing on effective pain and symptom management.
The Nurse Practitioner will collaborate with external referral sources to increase access to hospice services when the patient is deemed appropriate. This will include clarifying goals of care addressing end of life issues and care and transition planning into hospice care.
Reports to: Chief Medical Director
Essential Job Functions:
  • Provide comprehensive health assessments differential diagnosis and prescribe treatments and medication under the protocol in the direct management of acute and chronic illness and disease.
  • Perform Face-to-Face visits as required for certification and re-certification of patient eligibility; provide consultation and visits to patients at home long term care facilities inpatient facilities or other residence as needed.
  • Attend Palliative Interdisciplinary Group (IDG) meetings and facilitate discussion of patients allowing for full staff discussion consultation and evaluation.
  • Follow ethical and legal standards established by the medical and nursing communities as the basis for patient advocacy.
  • Maintain up-to-date patient records through EMR to assure patient problems plans actions and goals are accurately and clearly stated and changes are reflected as they occur including new information obtained during Palliative IDG meetings.
  • Ensure all documentation is completed within established agency guidelines and meets regulatory requirements.
Qualifications:
  • Must hold a current advanced practice license with appropriate certification in the State of Maryland
  • Must have advanced clinical assessment and pain management skills with knowledge of current palliative medicine standards and practices
  • Demonstrates critical thinking skills and the ability to exercise good judgment in emergency situations and ability to maintain effectiveness in stressful environments
  • Supports and identities with Compass philosophy of interdisciplinary approach to patient care planning and delivery
  • Successfully completes orientation
  • Flexible and able to work on several projects simultaneously
  • Able to work harmoniously with diverse individuals and groups
  • Willingness to advance knowledge and skills
  • Must attend in-services and workshops as appropriate
  • An ability to cope with the stress of continual loss
Physical Qualifications:
  • Possesses physical ability to perform job-related duties which may require lifting standing bending transferring stooping stretching walking pushing partial or complete assistance with activities of daily living without assistance from another health care worker or significant other.
  • Must be able to communicate verbally and in writing.
  • Must have strong computer skills proficient with EMR software and able to use a computer and other office equipment to document patient information.
  • Must be able to hear to process directions and requests from doctors employees patients and other individuals.
  • Must be able to drive to visit patients homes for supervision and patient care.
  • Must complete required vaccinations.
Schedule:
This is a part-time position working on-site 16 hours per week. Schedule to be determined.
