Job Summary
Human Touch Home Health is hiring a licensed Physical Therapist to provide home health services in Massaponax King George Fredericksburg Spotsylvania County and surrounding communities.

What Youll Do
- Complete physical therapy evaluations reassessments and discharge planning
- Develop individualized treatment plans and provide skilled therapy services in patients homes
- Help patients improve strength balance mobility and independence
- Coordinate care with physicians and the interdisciplinary team
- Complete documentation accurately and on time

Requirements
- Active Virginia Physical Therapist license
- Valid drivers license and reliable transportation
- Strong documentation and communication skills
