Position Summary

Navitas Healthcare LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and long-term care setting. This role focuses on delivering safe patient-centered care during overnight shifts including medication administration clinical monitoring and coordination with the interdisciplinary care team.

Key Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor residents conditions and promptly report any changes
• Perform routine nursing assessments and accurately document care provided
• Provide wound care and other basic nursing procedures within scope of practice
• Maintain complete and accurate clinical documentation and charting
• Assist with admissions discharges and transfers as needed
• Support Certified Nursing Assistants (CNAs) in resident care activities when required
• Ensure adherence to infection control and safety protocols at all times
• Respond to emergencies and resident needs during night shifts
• Collaborate effectively with the interdisciplinary healthcare team

Required Qualifications

• Active and unrestricted LPN license (Colorado or compact state eligibility)
• Prior experience in long-term care or skilled nursing preferred
• Ability to work consistent overnight 12-hour shifts
• Strong clinical judgment and documentation skills
• Ability to work independently and manage patient care during night shifts
